Assist the program participants to gain access to needed medical, social, educational and other community services. Provide service planning coordination, linkage/referral and follow-up/monitoring to program participants in an effort to facilitate independence in the community.

  • Develops Individual Service Plans (ISPs) with program participants based on identified interests and needs and person-centered planning.
  • Provides monitoring and coordination of follow-up services in regard to mutually agreed upon ISP objectives.
  • Provides on call case management services as indicated by program participant or program needs.
  • Assists program participants with arranging clinical assessments as required.
  • Coordinates on-going actions of person-centered planning team to ensure consistency in service plan implementation.
  • Coordinates/completes all necessary activities and reports as required by Service Coordination guidelines.
  • Documents all monthly and other contacts with program participants in case notes and other reports as necessary to meet funding requirements.
  • Required to attend a minimum of 15 hours of training annually during the first three years performing MSC duties and 10 hours annually thereafter.
  • Performs other duties as assigned by the Service Coordination Supervisor and/or Manager.

JOB REQUIREMENTS

  • Bachelor's Degree in Health or human Services or R.N.
  • At least 1 year of relevant experience.
  • Experience or knowledge of Windows, Word and Email preferred.
  • Must possess a valid driver's license and reliable transportation.
  • Must be able to tolerate the stresses associated with multiple, simultaneous demands.
  • Regularly required to talk, hear, stand, sit, walk and frequently required to use hand and fingers.
  • May occasionally lift and/or move up to 50 pounds.
